- Authenticated copies of the following:
- Certificate of Eligibility, Board/Bar Rating, and License (Two authenticated copies each).
- Transcript of Records (TOR).
- Certified True Copy of College Diploma.
- Original documents:
- Certificate of Live Birth.
- Certificate of Marriage (if married).
- NBI Clearance.
- Court clearances from the RTC and MTC, and Clearance from the Office of the Prosecutor where the applicant resides.
- Performance Rating from the last rating period (if applicable).
- Latest and duly filed Income Tax Return (ITR/W2) (for employed applicants).
- Latest and duly filed Statement of Assets, Liabilities, and Net Worth (SALN) (for government employees).
- Two (2) pieces of 2×2 colored ID pictures with a white background (must be taken without makeup or filters, in business attire).
